Sports Physicals Schedule

2009/10 Winter Sports begin 11/20/09 (first day of practice)

Students must have a current physical on file before attending practice. 

If you have participated in a fall sport your parents must complete "Section 5" of the PIAA form and

return the form to the school nurse.

• Private physician's physicals cannot be date prior to June 1, 2009. (Per PIAA Regulations)
• Students must pick up the necessary forms beforehand, in the main office to bring home for the parent/guardian’s to complete.  Please refer to the “Instructions for Sports Physicals”
